The Pelvic Health Fund is committed to respecting the privacy of our donors and the individuals we support. We are conscious of how we interact with personal information and strive to be transparent and accountable in maintaining the confidentially of any personal information we handle. For the individuals we support, we collect only information necessary to ensure they receive products that align with their pelvic health needs and the advice of their healthcare providers. For donors, we only collect information necessary to enable donations and reporting in compliance with state and federal guidelines. All information we receive will be stored securely and internally.

By using the Pelvic Health Fund website, using any of our online services, or communicating with Pelvic Health Fund through online/offline channels, you are deemed to have assented and agreed to the Pelvic Health Fund’s Privacy Policy as in effect at the time of your use.

The Pelvic Health Fund does not sell, trade, rent, or share the names, addresses, telephone numbers, or email addresses of our donors or the individuals we support.

We require parental consent before collecting, using, or disclosing personal information from children under the age of 13. If applying for products for pediatric use for an individual under the age of 13, we require a parent or legal guardian submit the application.

We neither ask for nor expect you to share any patient health information with anyone from the Pelvic Health Fund. Our only stipulation is that you must share whether you are currently under the care of a pelvic health related provider and provide their contact information as well as a letter of support from this provider. We require this information for safety reasons. Not everyone is appropriate for use of every product and we want to ensure you are safe in using any product we provide. Your provider should not include any information in the letter of support you attach to your product application other than your name and a statement of safety and need for a specific product. These forms will be stored securely and internally for a time period of 2 years. They shall never be disseminated externally.

We use cookies for a number of different purposes. Some cookies are necessary for technical reasons; some enable a personalized experience for both visitors and registered users; and some allow for support from authorized selected third party networks. Pelvic Health Fund does not sell, trade, or rent your personal information to other organizations or individuals in ways different from what is disclosed in this Privacy Policy.  Pelvic Health Fund is the sole owner of the information collected through its services and website and may use the information to assess Pelvic Health Fund’s programs and efforts for development. We do not disseminate any of your data externally other than is required for shipping purposes (name and address).

To respond to your requests and for updates about new content on the Websites and special events organized by Pelvic Health Fund, your email address in Pelvic Health Fund’s database will be used to communicate.  Pelvic Health Fund may also directly communicate with you via phone and/or direct mail concerning non-anonymous surveys, submission of content, or services we provide.  Information collected from the communication will be stored as long as Pelvic Health Fund deems reasonably necessary and practicable for the purpose of enhancing our services, or to otherwise meet legal requirements.

Pelvic Health Fund takes security seriously and will take reasonable technical and organizational precautions to prevent the loss, misuse, or alteration of your personal information both in transit and at rest. We mandate the use of HTTPS for all services using TLS (SSL), including our public website. We regularly audit the details of our implementation, including the certificates we serve, the certificate authorities we use, and the ciphers we support. We use HSTS to make sure that browsers interact with our website only over HTTPS. Information relating to online transactions entered via the website is encrypted using mutual transport layer security (mTLS) and has dedicated PGP keys for users to encrypt communications with.  However, there is no such thing as a 100% secure website.  Pelvic Health Fund cannot, and does not guarantee that unauthorized access, hacking, data loss, disclosure, misuse, modification, or any other breaches will not occur.

The content on our website may include links to external websites that are not directly operated and maintained by Pelvic Health Fund.  These external sites have their own privacy policies and/or data collection practices, which may be substantially different from Pelvic Health Fund’s, concerning personal information.  Pelvic Health Fund encourages you to review and learn about the privacy policies and practices of all external websites and services.  We take no responsibility for linked external sites.

The Pelvic Health Fund website is designed and targeted to users who reside in the United States and is governed by and operated in accordance with the laws of the United States. We make no representation that the website is operated in accordance with the laws or regulations of, or governed by other nations. By accessing and/or providing personal information through the website, you certify that you meet the legal requirements for disclosure of personal information in your jurisdiction.  If you do not meet the legal requirements, please contact us and request that we delete your personal information, and discontinue disclosing personal information through the Websites.
